In my hands, the best result is after a nipple sparing mastectomy in the proper candidate. That’s critical, it has to be the right patient and the right anatomy.

The nipple sparing mastectomy and immediate reconstruction has obvious aesthetic benefits due to preservation of the structures of the breast but limited patients are candidates. I’m a part of a nipple sparing registry that is performed on a national level because over and over again, multiple studies (some of which have been significant in volume both in the US and abroad) have reiterated the safety of this operation.

If during the mastectomy the nipple is spared, the breast cancer operation starts with tissue removal at the base of the nipple is removed and sent as a biopsy. Any nipple changes indicate removal of the nipple. If there are any cancer cells or even beginnings of changes on the frozen section, then the nipple is removed and will be reconstucted at a later date.

The advantage of maintaining the existing nipple and areola is that no further surgery is necessary create the nipple areola complex, but not every breast (or patient) is a candidate. This procedure should only be done if the breast cancer is small and distant from the nipple areola complex, the breast surgeon is experienced in this technique, and the nipple is in a good position for the reconstruction. Her result was spectacular on the table and it’s spectacular now. There’s still some minor work to be done but this can be achieved as long as the work is done properly FROM THE BEGINNING. I see many patients who come to me for a correction of previous work but this photo is not a fair one for them to anticipate. The patient did extremely well, the wounds are very young in this photo but now are completely healed. She’s healed and looks great. Good for her, and thank God!
